Оценить протективный эффект применения нагрузочной дозы аторвастатина на операционное повреждение, систолическую функцию миокарда и толерантность к физической нагрузке у пациентов со стабильной ишемический болезнью сердца (ИБС) при эндоваскулярной реканализации хронических атеросклеротических окклюзий коронарных артерий. Материал и методы. Обследовано 82 пациента. Пациентам в основной группе ( n = 38) назначалась нагрузочная доза аторвастатина 80 мг за сутки до операции. Пациенты контрольной группы ( n = 44) получали аторвастатин в дозе 20 мг. Определялся тропонин I, МВ-фракция креатинфосфокиназы (КФК), проводилась эхокардиография, велоэргометрия при поступлении, через 24 часа, через один месяц, через один год после операции. Результаты. Достоверных различий в исходном уровне тропонина I и МВ-КФК не выявлено. Через 24 часа после операции тропонин I в основной группе не повысился, в контрольной группе отмечено значимое его повышение. Через один месяц и один год в обеих группах не выявлено повышения уровня тропонина I. Перед операцией повышения уровня МВ-КФК не отмечено. Через 24 часа повышения МВ-КФК в основной группе не выявлено. В контрольной группе отмечалось значимое повышение МВ-КФК. Через один месяц и один год после операции показатели в двух группах не различались. Исходный уровень фракции выброса левого желудочка, конечного диастолического объема и конечного систолического статистически значимо не различались. Через 24 часа, один месяц и один год после эндоваскулярной реваскуляризации не было отмечено значимого изменения исследуемых параметров ни в одной из групп. Исходно уровень толерантности к физической нагрузке не отличался. Через 24 часа, один месяц и один год после операции значимого изменения исследуемого параметра не произошло ни в одной из групп. Заключение. Применение нагрузочной дозы аторвастатина в 80 мг перед реканализацией и стентированием хронической окклюзии коронарных артерий обладает протективным эффектом и позволяет снизить риск повреждения миокарда, ассоциированного чрескожным коронарным вмешательством. Высокая доза аторвастатина не оказывает значимого влияния на показатели центральной гемодинамики в ближайшем и отдаленном периоде.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To analyze the protective effect of atorvastatin loading dose on the reduction of myocardial injury associated with percutaneous coronary intervention, left ventricular systolic functionand exercise tolerancein patients with endovascular recanalization of chronic atherosclerotic total occlusions. Methods. 82 patients underwent recanalization. The patients in the main group ( n = 38) received a loading dose of atorvastatin 80 mg before surgery. Patients in control group ( n = 44) received 20 mg of atorvastatin. In both groups Troponin I, CF fraction of creatine phosphokinaseweremeasured, echocardiographyand veloergometer test were performed prior to surgery, 24 hours, 1 month, 1 year after the surgery. Results. There was no difference in Troponin I and CF fraction of creatine phosphokinase initial levels. In 24 hours in the main group in 3 patients (7,9 %) there was discovered moderate troponin I rising. In the control group there were 11 (25 %) patients with increasing in troponin I level in 5 and more times. CF fraction of creatine phosphokinase in control group was increased in 3 (7,9 %) patients, in the control group this parameter was increased in 16 (36,4 %) patients. In 1 month and 1 year Troponin Iand CF fraction of creatine phosphokinaselevel was normal in the both groups.There were no statistically significant changes in echocardiography parametersor exercise tolerance during the all follow-up period. Conclusion. Atorvastatin loading dose has a protective effect and prevents periprocedural myocardial injury.
