В исследование включено 423 последовательно госпитализированных пациента в Кемеровский кардиологический диспансер по поводу инфаркта миокарда с подъемом сегмента ST (ИМпST). Через год у 397 (95,4 % от исходного количества выписанных больных) пациентов определен исход заболевания. Зарегистрировано 39 смертельных случаев.</p></sec><sec><title>   Результаты</title><p>   Результаты. Частота выявления мультифокального атеросклероза (МФА), определяемого при утолщении комплекса интима-медиа общей сонной артерии или наличии стенозов любой степени в экстракраниальных артериях или артериях нижних конечностей оказалась крайне высокой – 95 %. Установлено, что с увеличением степени стенозов периферических артерий достоверно повышается частота выявления многих факторов кардиоваскулярного риска. У пациентов без МФА (n = 18) смертельных случаев в течение года не было, у больных со стенозами экстракраниальных артерий или артерий нижних конечностей менее 30 % (n = 193) смертность составила 5,7 %, а со стенозами более 30 % (n = 186) – 15,1 % (χ2 = 11,4, р = 0,003). Использование шкалы GRACE совместно с идентификацией дополнительных факторов (наличия застойной сердечной недостаточности, МФА, факта ранее перенесенного инсульта, курения) позволяет создать модель (Kem Score) стратификации риска смерти в течение 12-месячного периода со значением площади под ROC-кривой 0,83 (0,76; 0,90), что существенно выше первичной шкалы GRACE – 0,71 (0,63; 0,79).</p></sec><sec><title>   Заключение</title><p>   Заключение. Выявление дополнительных факторов риска, включая стенозы периферических артерий от 30 % и выше, позволяет оптимизировать прогностическую ценность шкалы GRACE.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Aims</title><p>   Aims. To evaluate the clinical and prognostic role of hemodynamically insignificant stenosis of the extracranial arteries and lower extremity arteries in patients after myocardial infarction with ST – Seg ment elevation.</p></sec><sec><title>   Methods and results</title><p>   Methods and results. The study sample consisted of 423 consecutively admitted patients in Kemerovo Cardiological Clinic with ST-Segment elevation myocardial infarction (STEMI). The disease outcome was defined in 397 patients (95.4 % of the initially discharged patients) a year later. 39 fatal outcomes were registered.</p></sec><sec><title>     Results</title><p>     Results. The incidence of polyvascular diseases (PVD), defined by the intima-media thickness of the common carotid artery or presenting stenosis of any severity of the extracranial arteries (ECA) or lower extremity arteries (LEA) was substantially high – 95 %. The increasing severity degree of peripheral arteries stenosis was noted to raise significantly the cardiovascular risk factors incidence. In patients without PVD (n = 18) no fatal outcomes were registered, in patients with the ECA or LEA stenosis – &gt;30 % (n = 193), the mortality rate was 5.7 %, and with stenosis &lt; 30 % (n = 186) – 15,1 % (χ2 = 11,4, p = 0.003). Using the GRACE score model together with the additional factors determination (presenting congestive heart failure, PVD, prior stroke, smoking) enables to create a scheme (KemScore) of death risk stratification for 12-month period, with the value of the area under the ROC-curve 0.83 (0.76, 0.90), that is significantly higher than the initial GRACE score – 0.71 (0.63, 0.79).</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Additional risk factors determination, including peripheral arterial stenosis of 30 % and more, enables to optimize the predictive GRACE score model value.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>острый коронарный синдром</kwd><kwd>мультифокальный атеросклероз</kwd><kwd>GRACE</kwd></kwd-group><kwd-group xml:lang="en"><kwd>acute coronary syndrome</kwd><kwd>polyvascular diseases</kwd><kwd>GRACE score model</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Morrow D.
