Оценить протективный эффект аторвастатина при эндоваскулярной реваскуляризации хронических окклюзий коронарных артерий у пациентов с различной тяжестью атеросклероза коронарных артерий. Материал и методы. В исследование вошли 38 пациентов, подвергшихся реканализации хронических окклюзий коронарных артерий. Всем пациентам назначалась нагрузочная доза аторвастатина 80 мг за сутки до операции. Далее пациенты были разделены на группы высокого и низкого риска по шкале SYNTAX. Определялся тропонин I и МВ фракция креатинфосфокиназы, оценивалось функциональное состояние миокарда с помощью эхокардиографии за одни сутки до, через 24 ч и через один месяц после чрескожного коронарного вмешательства. Результаты. В группе пациентов высокого риска через 24 ч после реканализации окклюзии был определен диагностически более высокий уровень тропонина I. В группе низкого риска уровень тропонина I через сутки после реваскуляризации был значимо меньше. Достоверных изменений уровня МВ-КФК не произошло ни в одной из групп. Статистически достоверного изменения эхокардиографических параметров не было выявлено ни в одной из групп в течение всего периода наблюдения. Заключение. Результаты оценки уровня кардиомаркеров показали, что нагрузочная доза аторвастатина в 80 мг перед реканализацией и стентированием хронических окклюзий коронарных артерий обладает протективным эффектом и предотвращает развитие периоперационного повреждения миокарда. Аторвастатин в нагрузочной дозе 80 мг оказывает протективный эффект только у пациентов с нетяжелым атеросклерозом, т.е. с баллами по шкале SYNTAX менее 19,5. При этом высокая доза аторвастатина не оказывает значимого влияния на показатели центральной гемодинамики в ближайшем и отдаленном периодах.</p></abstract><trans-abstract xml:lang="en"><p>The study objective is evaluate atherosclerosis burden on atorvastatin protective effect in patients with chronic total occlusion recanalization. Methods. 38 patients underwent recanalization of chronic total occlusions. All patients received a loading dose of atorvastatin 80 mg before surgery. Further patients were divided into high and low risk group according to SYNTAX score. In both groups, troponin I, CF fraction of creatine phosphokinase, functional parameters of the myocardium were measured 1 day prior to surgery, 24 hours and 1 month after the surgery. Results. In the high risk group there was discovered more significant troponin I rising compared with low risk group. In the low risk group troponin I level was significantly lower. CF fraction of creatine phosphokinase in both groups was similar. There were no statistically significant changes in echocardiography parameters. Conclusion. Atorvastatin loading dose has a protective effect and prevents periprocedural myocardial ischemia. At the same time this effect appears only in patients with low atherosclerosis burden - SYNTAX score less than 19.5.
