Предполагается, что NK-клетки могут участвовать в формировании и прогрессировании атеромы путем распознавания липидов и продукции интерферона-γ, перфорина и гранзима Б.</p><p>Цель исследования – изучить особенности субпопуляционного состава NK- и NKT-клеток и их диагностическую ценность у пациентов среднего возраста в зависимости от наличия субклинического атеросклероза артерий нижних конечностей.</p><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 80 человек (44 мужчины (55 %) и 36 женщин (45 %)), возраст которых составлял 49 (44,0; 56,0) лет. Всем пациентам проводили дуплексное ультразвуковое сканирование (ДУС) артерий нижних конечностей. Иммунофенотипирование лимфоцитов выполняли на проточном цитометре Navios 6/2 (Beckman Coulter, США) с использованием меченых мышиных антител: СD45-PC7.0, CD16-PE, CD11b-FITC, CD4-APC, CD8-PC5.5, CD3-ECD производства компании Beckman Coulter, США.</p></sec><sec><title>Результаты</title><p>Результаты. По результатам ДУС артерий нижних конечностей пациенты разделены на две группы: в первую группу включены 46 (57,5 %) пациентов с атеросклерозом артерий нижних конечностей, во вторую – 34 (42,5 %) пациента без атеросклеротических бляшек в артериях нижних конечностей и с лодыжечно-плечевым индексом ≥0,9. У больных первой группы в крови было статистически значимо больше, чем у лиц второй группы, циркулирующих NK-клеток (152 (102; 203) кл/мкл против 118 (80,0; 146) кл/мкл; p = 0,048) и меньше NK-CD4+CD11b+-клеток (3,00 (0,00; 15,0) кл/мкл против 25,0 (5,00; 72,0) кл/мкл; p = 0,035). Снижение количества NK-CD4+CD11b+-клеток ≤16,5 кл/мкл позволяло диагностировать атеросклеротическое поражение артерий нижних конечностей с чувствительностью 80,0 % и специфичностью 58,8 %. По данным логистического регрессионного анализа уменьшение их содержания ≤16,5 кл/мкл ассоциировалось с отношением шансов поражения артерий нижних конечностей 5,71 (95%-й доверительный интервал 1,44–22,6; p = 0,013).</p></sec><sec><title>Заключение</title><p>Заключение. Пациенты с атеросклерозом артерий нижних конечностей отличались статистически значимо большим, чем у лиц группы сравнения, количеством циркулирующих NK-клеток и меньшим – NK-CD4+CD11b+-клеток. Снижение содержания последних ≤16,5 кл/мкл позволяло диагностировать атеросклеротическое поражение артерий нижних конечностей с чувствительностью 80,0 % и специфичностью 58,8 %.</p></sec></abstract><trans-abstract xml:lang="en"><p>Aim of the study was to investigate the features of the subpopulation composition of circulating NK and NKT cells and their diagnostic value in middle-aged patients, depending on the presence of subclinical atherosclerosis of lower limb arteries.</p><sec><title>Material and methods</title><p>Material and methods. The study included 80 patients (44 men (55.0 %) and 36 women (45.0 %)), whose age was 49.0 (44.0; 56.0) years. All patients underwent duplex ultrasound scanning (DUS) of the arteries of the lower extremities. Lymphocyte immunophenotyping was performed on a Navios 6/2 flow cytometer (Beckman Coulter, USA) using labeled mouse monoclonal antibodies CD45-PC7.0, CD16-PE, CD11b-FITC, CD4-APC, CD8-PC5.5, CD3-ECD manufactured by Beckman Coulter, USA.</p></sec><sec><title>Results</title><p>Results. According to the results of the DUS of the arteries of the lower extremities, the patients were divided into two groups: the first group included 46 (57.5 %) patients with lower extremity artery atherosclerosis, the second group included 34 (42.5 %) patients without plaques in lower extremity artery. Patients of first group had significantly higher, than patients of second group, number of circulating NK cells and lower number of CD4+CD11b+ NK cells. The decrease in the CD4+CD11b+ NK cell number ≤16.5 cells/μl allowed to diagnose lower extremity artery atherosclerosis with a sensitivity of 80.0 % and a specificity of 58.8 %. According to logistic regression analysis, a decrease in the number of CD4+CD11b+ NK cells ≤16.5 cells/μl was associated with a odds ratio of lower extremity artery atherosclerosis 5.71 (95 % confidence interval (CI) 1.44-22.6; p = 0.013).</p></sec><sec><title>Conclusion</title><p>Conclusion. Compared with the control group, patients with lower extremity artery atherosclerosis differed in a statistically significantly larger number of circulating NK cells and a smaller number of CD4+CD11b+ NK cells.
