Оценить связь между уровнем потребления алкоголя и параметрами липидного обмена среди взрослых мужчин.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включен 881 мужчина в возрасте 35–69 лет из общего населения г. Архангельска, которые приняли участие в исследовании «Узнай свое сердце» в 2015–2017 гг., и 161 мужчина, получавший стационарное лечение с диагнозами, связанными с употреблением алкоголя (наркологические пациенты). Участники были разделены на пять уровней потребления алкоголя: непьющий, беспроблемный (нечастное умеренное потребление), опасный (частое потребление в опасных для здоровья дозах), вредный (преднозологический) и наркологические пациенты. С помощью многомерных линейных регрессий мы оценили различия между этими группами по уровню атерогенных липидов (общий холестерин (ХС), триглицериды (ТГ), липопротеины низкой плотности (ЛПНП), аполипопротеин В (апо В), липопротеин (а) (ЛП(а)), остаточный ХС, ХС, не входящий в состав липопротеинов высокой плотности (не-ЛПВП)), антиатерогенных липидов (ЛПВП, аполипопротеин A1 (апо А1)) и соотношения АпоB/АпоA1.</p></sec><sec><title>Результаты</title><p>Результаты. В сравнении с беспроблемно пьющими у опасно пьющих уровень ХС был в среднем выше на 0,22 ммоль/л, ЛПВП – на 0,07 ммоль/л, апо B – на 0,04 г/л, у вредно пьющих содержание ЛПВП и апо A1 было больше на 0,15 и 0,08 г/л соответственно, а соотношение апо B/апо A1 меньше на 0,06. У наркологических пациентов концентрация ХС была в среднем ниже на 0,42 ммоль/л, ЛПНП – на 0,41 ммоль/л, апо B – на 0,09 г/л, не-ЛПВП – на 0,45 ммоль/л, отношение апо B/апо A1 – на 0,08, уровень ТГ был выше на 0,15 ммоль/л; содержание ЛП(а) выше на 0,29 мг/дл только после коррекции на маркеры функции печени. У непьющих уровень ХС был в среднем ниже на 0,29 ммоль/л, ЛПВП – на 0,11 ммоль/л, апо A1 – на 0,08 г/л.</p></sec><sec><title>Заключение</title><p>Заключение. В сравнении с беспроблемно пьющими у лиц с опасным потреблением алкоголя повышено содержание атерогенных и антиатерогенных липидных фракций, у вредно пьющих – только антиатерогенных липидов, у наркологических пациентов был самый низкий уровень атерогенных липидов на фоне повышения концентрации ТГ. Следовательно, липидный профиль может отражать потребление алкоголя, что следует учитывать при оценке сердечно-сосудистых рисков.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the association between levels of alcohol consumption and lipid metabolism parameters among adult men.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 881 men aged 35–69 from the general population of Arkhangelsk, who participated in the 2015–17 Know Your Heart study, and 161 men who received inpatient treatment for alcohol-related diagnoses (narcology patients). Participants were divided into five levels of alcohol consumption: non-drinkers, non-problem (infrequent moderate consumption), hazardous (frequent consumption in doses hazardous to health), harmful (prenosological), and narcology patients. Using multivariate linear regressions, we analyzed differences between these groups in atherogenic lipid fractions (total cholesterol [TC], triglycerides [TG], low-density lipoprotein [LDL], apolipoprotein B [ApoB], remnant cholesterol, and non-HDL cholesterol, lipoprotein(a)) (Lp(a)), antiatherogenic lipid fractions (high-density lipoprotein [HDL], apolipoprotein A1 [апо A1]), and in ApoB/апо A1 ratio.</p></sec><sec><title>Results</title><p>Results. Compared with non-problem drinkers, hazardous drinkers had higher mean levels of HDL by 0.22 mmol/L, HDL by 0.07 mmol/L, and ApoB by 0.04 g/L. Among harmful drinkers, mean HDL was higher by 0.15 mmol/L and апо A1 by 0.08 g/L, but the ApoB/апо A1 ratio was lower by 0.06. Among narcology patients, mean TC levels were lower by 0.42 mmol/L, LDL by 0.41 mmol/L, ApoB by 0.09 g/L, ApoB/апо A1 by 0.08, and non-HDL by 0.45 mmol/L, but TG was higher by 0.15 mmol/L. Lp(a) in this group was higher by 0.29 mg/dl only after adjustment for markers of liver function. Non-drinkers had on average lower levels of TC by 0.29 mmol/L, HDL by 0.11 mmol/L, and апо A1 by 0.08 g/L.</p></sec><sec><title>Conclusions</title><p>Conclusions. Compared with non-problem drinkers, hazardous drinkers had elevated levels of both atherogenic and antiatherogenic lipid fractions, hazardous drinkers had only elevated levels of antiatherogenic fractions, and narcology patients had the lowest levels of atherogenic lipid fractions but elevated TG levels.
